School Health Index Assessment

The SHI is a self-assessment and planning tool that schools can use to improve their health and safety policies and programs. It was created by the US Centers for Disease Control and Prevention/ Division of Adolescent and School Health (US CDC/DASH). The Maine Coordinated School Health Program and the Healthy Maine Partnership wishes to thank them for allowing us to modify it to meet our state needs.
