School Nurse Surveillance Site

School Nurse Tool
To enter the School Nurse Tool click the School Nurse Tab in the menu bar and enter your School Nurse login and password.

To change School information, such as mailing address or phone number:

  1. Click in the appropriate text box,
  2. Edit/correct the text
  3. Click Update.
Vision, Hearing, and Spinal Reports
To enter data into the Vision, Hearing and Spinal Reports:
  1. Click on the appropriate grade level button located under the blue Vision, Hearing, and Spinal Report line, for which you are reporting. The sample buttons are names Grade 1 and Grade 3.
  2. Click Create Report to create a report for the grade and school selected. This report will be where you will enter your data for the grade selected.
  3. Once the report is created, click Enter Report to open a School Health Screening Report Form.
  4. Click in the first text box, add the data, and tab through the text boxes adding all available data.
  5. When all data has been entered, click Add Vision, Hearing and Spinal Data; the report will refresh, with all data filled in and percentages calculated.
  6. Review your data to be sure of accuracy.
  7. Make any changes necessary and click Add Vision, Hearing and Spinal Data, again.
  8. Once all data is verified as correct, click Return to Directory to return to the school directory page. You need to repeat this process for each school/grade for which you report surveillance data.
Other Data
The "All Grades" Report is for reporting insurance, medication, chronic disease, complex nursing interventions, and acute visits data.

To enter data into the "Other Data" Report:

  1. Click on the All Grades button located under the blue Other Data line;
  2. Check the boxes for the reports sections for which you will submit data,
  3. Click Create Report to create a report,
  4. Click in the first text box; add the data and tab through the text boxes to complete the first form
  5. Click the appropriate button for access to the following report forms:
    • Medications
    • Chronic Diseases
    • Complex Nursing Interventions
    • Acute Visits
  6. Once all data is verified as correct, click Return to Directory to return to the school directory page.
District Tool
Aggregated data for School-wide surveillance reports and for Vision, Hearing and Spinal surveillance reports is available in the District Reports area.

School-wide Report
To view the school-wide report, click on the option list and select a report group, then click View Report. The demographic data will be displayed along with the PCP and insurance data. Additional surveillance report forms are available and data is viewed the same way. Click the appropriate button for access to the following report forms:

  • Medications
  • Chronic Diseases
  • Complex Nursing Interventions
  • Acute Visits
Vision, Hearing, and Spinal Reports
To view the Vision, Hearing, and Spinal Reports, click on the option list and select a report group, click on the grade level option list and select a grade level, then click View Report. The demographic data will be displayed along with the data for all vision, hearing, and spinal reports, according to the options you selected.

School Account Tool
The School Account Tool can be used to set up and maintain school records. Click School Account Tool in the sub-menu to view school information and to add/edit schools.

Adding a School
To add a school to the School Account Tool click Add School. Type all demographic information into the text boxes provided. Be sure to add the school code. Select the appropriate grade levels for the school VHS surveillance reports by selecting yes/no from the option list near each grade level. (Select Yes to include the grade for the school.) Complete the process by clicking Add School.

Editing School Information
To edit school information click Edit School Info in the row for the selected school. Click in the text box and retype the information you wish to change or re-select from the option lists. Complete the process by clicking Submit Changes.

School Nurse Accounts
The District Account Tool can be used to set up and maintain all logins/passwords for the district. Click User Account Tool in the sub-menu to add, view, or edit login/password information.

Adding a New School Nurse
Add User and enter Last Name, First Name, Login, Password (twice, for verification). Click Add User to complete the process.

Editing a School Nurse Information
To edit the Last Name, First Name, Login, or Password of a current school user, click in the text box for the item you wish to change, edit the text and click Edit. The screen will refresh with the correct data.

Giving a School Nurse Access to a School's Reports
To assign school report access to a user click in the text box labeled Current User ID for the selected school type (or retype to edit) the User ID assigned to the user in the section labeled Current District Accounts and click the "Assign USER ID to School" button.

Basic Skills
The School Nurse Surveillance website requires basic web browsing skills.

· Using a Web Browser
Internet Explorer and Netscape Communicator have been tested and both work well for viewing all content on the School Nurse Surveillance website. Both browsers perform the same function and the choice of which to use is a personal preference to the end user. The browser you use determines how the School Nurse Surveillance website will look and work. For example, the print button in Internet Explorer may print the document immediately but the print button in Netscape Communicator may open a print dialogue box - both however allow you to print. It is recommended that you use the latest browser version of your preferred choice. Features in both browsers have been improved as newer versions have been introduced allowing more security and ease of use.

Both Windows and Macintosh computers have been tested and work with the website.

Navigation Menu

The website has a horizontal navigation menu at the top of every screen. Click any tab in the menu to move to the selected location in the School Nurse Surveillance website.

The navigation menu sometimes has a smaller bar beneath it with bulleted features. Clicking on a bulleted feature will take you to the indicated location.

Click on each tab in the Navigation Menu Bar to view the following:

Home -the website's public home page
Documentation -the website's documentation area
Resources -the website's project resources area
School Nurse -the School Nurse Login Page: an area for the school nurse to enter a login and password to gain access to the School Nurse Tool.
District Tool -the District Tool Login Page: an area for the District Administrator to enter a login and password to gain access to the District Administration Tool
State Tool -the State Tool Login Page: an area for the State School Nurse Surveillance Administrator to enter a login and password to gain access to the State School Nurse Surveillance Administration Tool

Pop-Up Window

The pop-up window is a separate, smaller window in the foreground of your work containing useful information. The Web Assessment Tool utilizes pop-ups throughout the website, which are initiated with a single click on well-placed links. This allows the viewer to gain access to the information without losing their working website location.

The pop-up window can be closed by clicking the "X" in the top right corner or by clicking the "CLOSE" link at the bottom of the pop-up.

Online Form Design

The Tool uses Online Forms to collect data.

The forms have these fields:

  1. Text boxes, which allow narratives
  2. Text option lists, which force the selection of text from an options list
  3. Radio buttons, which force the selection of one item from a list
  4. Check boxes, which allow the selection of one or more items from a list
Printing

Printing pages from the website works just like printing any other page, and varies according to the browser being used. It is recommended to set your margins at ½" instead of the standard browser default of ¾". This can be changed in the browser's page setup area.
