To enter your School Nurse login and password in the box above. If you do not know your password or need to have changes made to the schools for which you report, please call the Help Desk at 624-6672 or email rebecca.minton@maine.gov |
| Vision, Hearing, and Spinal Reports |
Select the School (If more than on School);
Select the grade level. The button is under the blue Vision, Hearing, and Spinal Report line;
Create Report to enter your data;
Click Enter Report to open the report form;
Enter the data in the boxes;
Click Add Vision, Hearing and Spinal Data;
The window will refresh, with all data filled in and percentages calculated.
Review your data to be sure of accuracy.
Once all data is verified as correct, click Return to Directory;
Repeat this process for each school/grade.

| Other Data |
The Other Data Report covers insurance, medication, chronic disease, complex nursing interventions, and acute visits data.
To enter data into the "Other Data" Report:
- Click on the All Grades button;
- Check the boxes for the reports sections for which you will submit data;
- Click Create Report to create a report;
- Enter the data in the boxes;
- Click the appropriate button for access to the following report forms:
- Medications;
- Chronic Diseases;
- Complex Nursing Interventions;
- Acute Visits;
- Once all data is verified as correct, click Return to Directory.
