About the Coordinated School Health Program Survey
The Coordinated School Health Program Survey, adapted by the Maine Department of Education from work done by David Lohrman of the Academy for Educational Development, asks questions about the health programs in your school and is intended to determine the extent and quality of these programs. Survey results will be used to describe the present status of health programs in your school, identify priority areas, and form a school health improvement plan.
Your school district has programs that address student and staff health. Sample Programs may be Youth, Parent, Family, Community Involvement, Comprehensive School Health Education, Physical Education & Physical Activity, School Counseling, Physical & Behavioral Health Services, Nutrition Services, Health Promotion & Wellness, Physical Environment, and School Climate. The survey questions ask about some characteristics of health program coordination in your school district. (Coordination means exchanging information, sharing resources, and altering activities for mutual benefit and to achieve a common goal.)
