Survey Explanation / Purpose

This survey asks questions about the health programs in your school and is intended to determine the extent and quality of these programs. Survey results will be used to describe the present status of health programs in your school, identify priority areas, and inform a school health improvement plan. Please provide your impressions of the CSHP in your school by completing the survey.

Your school district has some programs that address student and staff health such as: Youth, Parent, Family, Community Involvement, Comprehensive School Health Education, Physical Education & Physical Activity, School Counseling, Physical & Behavioral Health Services, Nutrition Services, Health Promotion & Wellness, Physical Environment, and School Climate. The following questions ask about some characteristics of health program coordination in your school district. (Coordination means exchanging information, sharing resources, and altering activities for mutual benefit and to achieve a common goal.)
