CSR Surveys for Students and Faculty
Spring 2007
Purpose
The purpose of this anonymous survey is to collect information about opinions and perceptions related to what is happening in your building regarding your school and its improvement efforts. Your responses to the survey questions will provide important and useful feedback to your school community. Ultimately, the feedback gathered will help your school make better plans for continuous improvement.

Tips for Taking the Survey

  • To avoid missing any questions, answer the questions in sequential order.
  • The Student and Faculty Survey can be completed with adequate thoughtfulness in 25-30 minutes, although it may take longer if numerous comments are written.
  • Your survey will be available for 5 hours from your start time. Following any interruption during those 5 hours, you will be able to login again and return to any unanswered questions.
  • While taking the survey, do not use your browser's back button. Using the back button may result in loss of all completed data, or failure for your data to reach the compiled results of the survey.
  • If you have changed your mind, or have made an error you will need to contact Gary Seekins
Survey Design
This survey is being administered to several of Maine middle schools, and it attempts to address the major school improvement efforts underway in these schools. If you come across statements about practices not currently being implemented please respond or check "don't know/not applicable".

Response Choices
Most of the items in this survey are statements of opinion or perception about the school. You will have several choices when responding to each of these statements:
  Strongly Agree
Agree
Somewhat Agree
Somewhat Disagree
Disagree
Strongly Disagree
Don't Know/Not Applicable

The "Don't Know/Not Applicable" choice is probably the best choice if:
  • You don't think the statement applies to your school.
  • You have no opinion on the statement, or you don't understand the statement.
Demographic Questions
In order to set priorities for school improvement that take the needs and preferences of different groups into consideration, it's important to see if people with different experiences have different opinions or perceptions about the school. To make this possible, there are several questions at the end of the survey about the experiences of each person completing the survey. We hope you will complete these questions, but if you feel uncomfortable with any of these questions please leave them blank. Please note that all the information collected will remain anonymous.

Length of the Survey
Surveys are being given to faculty and students, with both versions containing numerous "common" items. Experience indicates that the Student and Faculty Surveys can be completed with adequate thoughtfulness in 25 -30 minutes, although it may take longer if numerous comments are written. If you do not have this block of interrupted time, your survey will be available for 5 hours from your start time. You will be able to login and return to any unanswered questions.
